The Universal Personalised Care (UPC) strategy actions 8 and 9 focus on the recruitment and training of social prescribing link workers and helping build the capacity of communities to support the wellbeing of local people. Other ‘connector’ or ‘link worker’ models are operating in most areas and many organisations and initiatives are concerned with community capacity building in place.
All are warmly welcomed to this webinar hosted by C4PC, launching this important report on an NHS England-commissioned piece of place-based work in York and Wakefield – testing and analysing effective ways to understand and align link worker and community strengthening activity.
