Welcome to the
Coalition for Personalised Care
The Coalition for Personalised Care is the single largest partnership of people and organisations in the country which comes together solely to focus on personalised health and care.
We believe that care based on what matters to people, is what matters most and we are committed to ensuring that personalised care is a reality for everyone using health and care services in England and Wales.
The breadth and diversity of our network helps us to gather unique insight, share learning, highlight challenges and encourage change. We invite you to find out more about how we work and our vision for the future.
Our growing network spans every facet of health and social care. There are a number of ways that you can get involved, either as an individual or an organisation.
As part of our successful drive to grow and diversify, we’re pleased to welcome the Pernicious Anaemia Society (PAS) as a new partner of the Coalition. The charity was created by Martyn Hooper MBE after his diagnosis with the condition and, in this interview with C4PC, he tells his story.
“The Coalition is in a strong, unique position to bring about change. As a person with lived experience, it is a place that gives equal weight and value to my contribution, providing me with a great platform from which to help and promote personalised care.”
