Welcome to the
Welcome to the
Coalition for Personalised Care
The Coalition for Personalised Care is the single largest partnership of people and organisations in the country which comes together solely to focus on personalised health and care.
We believe that care based on what matters to people is what matters most, and we are committed to ensuring that Personalised Care is a reality for everyone using health and care services in England. Our strong strategic relationship with NHS England and the Personalised Care Group enables us to work closely together towards our shared goal of Personalised Care becoming universal across health and social care.
The breadth and diversity of our network help us to gather unique insight, share learning, highlight challenges and encourage change. We invite you to find out more about Personalised Care and our Partner organisations.
We want to support people to think differently about what Personalised Care means in their environment and help them to take the practical steps needed to make it possible.
We do this by:
Providing trusted learning resources and sharing our knowledge and expertise.
Promoting evidenced-based approaches which are centred on what matters to people.
Championing Personalised Care by sharing best practice and empowering people with the skills, knowledge and confidence to benefit from it.
Providing hands-on support to encourage people, organisations and communities to think differently about Personalised Care and create the conditions for change.
Building stronger communities and supporting people already making a difference.
Ensuring that services are co-produced with people who have experience of using them.
Using our collective, independent voice to advocate for and shape change.
Understanding the reality of Personalised Care in a way which can shape strategy and plans for the future.
We are very pleased to welcome KeyRing as the newest Partner in the Coalition for Personalised Care. KeyRing provides adult social care that inspires people to build the life they want in their local community. They deliver flexible, personalised support that helps people smash barriers to independence.
